Nowadays there are more then 30 approved medicines for treatment epilepsy. Modern medical scientific society continuing investigate new generations of antiepileptic medicines with higher efficiency and lesser amount of side effects. Despite this good facts 30 % of patients suffering from epilepsy are still resistant to pharmacological therapy. For such kind of patients, especially who have generalized or unknown epilepsy neuromodulation therapy could be a good choice, one of them is vagus nerve stimulation (VNS).

In this review, we provide an literature overview on VNS system, it’s mechanism of work, effectiveness of different types and rates of use, possible side effects and future opportunities for the implementation of VNS system into clinical practice.

After analyzing the effectiveness of SWT and vitamin D in the complex treatment of patients with dorsalgia, we came to the conclusion that the use of SWT is safe and effective, has an analgesic systemic effect, which normalizes local microcirculation and makes it possible to reduce the dose of prescribed drugs, reducing the risk of side effects of NSAIDs. in patients with dorsalgia of inflammatory origin.

The acute toxicity and psychopharmacological activity of 1-(2-hydroxyphenyl)-6,7-dimethoxy-1,2,3,4-tetrahydroisoquinoline at doses from 1 to 100 mg/kg was studied. In doses of 10 and 20 mg/kg, intragastric administration revealed sedative-anxiolytic properties.

In recent years, there has been an increase in the frequency of placental insufficiency (PI) in pregnant women with obstetric and somatic pathology, including in patients with varicose veins, especially in the presence of autoimmune antibodies of various etiologies. The antiphospholipid syndrome (APS) attracts more and more attention of scientists, which significantly increases both maternal and perinatal mortality [8, 9]. Most of the adverse outcomes in APS are associated with PI. One of the reasons for the development of PI are disorders of the hemostasis system in pregnant women with the presence of circulating antiphospholipid antibodies (APA). AFAs circulating in the blood plasma contribute to the activation of the platelet link of hemostasis, disruption of the hemostatic potential, and potentiation of endothelial damage. Such changes lead to local ischemia of the villous chorion, impaired placental blood flow. The consequences of pathological changes in PI in the fetoplacental system lead to hypoxia, fetal malnutrition, increase perinatal mortality, and also have adverse consequences for the development of the child [2,8,9]. Risk factors for the development of fetal malnutrition are numerous. These include various somatic diseases, disorders in the reproductive system, pregnancy complications, social factors and much more. According to the literature, in women with an autoimmune cause of miscarriage without treatment, up to 90% of pregnancies are interrupted, and the effectiveness of treatment with the most modern approaches reaches 80% or more [5, 10].

Pregnancy is a favorable condition for the implementation of the pathogenic action of AFA. The manifestation of pathological processes can occur at different times, starting from the moment of conception: the process of implantation and early embryogenesis are disrupted. According to the literature, AFAs are capable of disrupting several aspects in the process of trophoblast differentiation, which is expressed in a change in the adhesiveness of the embryo, impaired fusion of syncytium, and a decrease in the depth of trophoblast invasion; a decrease in the production of hCG, an increase in thrombotic tendencies. As a result, these changes lead to a decrease in the protein-synthesizing and hormonal functions of the placenta. In the absence of adequate therapy, thrombosis occurs in the microcirculatory bed, which causes PI, chronic hypoxia, and often fetal death due to acute circulatory disorders in the vessels of the placenta [5, 6, 10].

Among patients with PN, 16-25% of women have manifestations of varicose veins, chronic venous insufficiency and varicose veins of the pelvis. Some authors attach significant importance in the occurrence of varicose veins in pregnant women to immunological mechanisms that lead not only to functional, but also to structural changes in the vessels. As a result, a cascade of pathological changes occurs, initiated by venous stasis and hemocoagulation abnormalities, which lead to PI. Treatment of this category of patients presents significant difficulties, as it can lead to the most severe complications in obstetrics, thrombosis and pulmonary embolism [1, 3].

In this article, we studied an anticonvulsant model induced by different analyzers of corazole (80 mg/kg s.c.), strychnine (1.2 mg/kg s.c.), isoniazid (300 mg/kg ip) and bicuculin (2.7 mg/kg s.c.) when administered orally to white mice, N-deacetylappaconitin at doses of 0.01; 0.05; 0.1; 0.5; 1.0 and 2.0 mg/kg anticonvulsant effect of the test substance, comparative drug carbamazepine and control groups. The anticonvulsant effect of N-deacetylappaconitine, carbamazepine and the control group at various doses was studied and the necessary conclusions were drawn.

To summarize the accumulated experience and compare the available materials for plastic surgery of dura mater (DM) defects.

An increasing number of patients who have suffered a traumatic brain injury and an increasing number of neurosurgical operations for tumor processes, congenital malformations leads to an increase in the number of DM defects and related complications. Despite the development of technologies for obtaining highly effective medical devices, the percentage of postoperative liquorrhea is still high, with subtentorial localization of the defect reaches 32%. Complications such as pseudomeningocele, postoperative inflammatory processes, liquorrhea, implant rejection, scar-adhesive processes leave unresolved the question of finding a suitable substitute for DM defects. The article presents an overview of the main materials for closing defects of DM: autografts, biological, synthetic materials. Their positive and negative qualities are discussed depending on the type and localization of damage, the type of material used. The analysis of the main characteristics that an ideal substitute for DM should correspond to is carried out. Composite materials are considered as a promising direction for the development of modern bioengineering.

The effect of the flavonoid glycoside sophoraflavonoloside isolated from the petals of the Crocus sativus L plant (Saffron seed) on hepatoprotective, choleretic and approximate motor activity in chronic liver damage with heliotrin when administered orally at a dose of 100 mg / kg under experimental conditions was studied. The results obtained on the basis of the conducted studies were compared with the control group and the most active dose of the comparative drug Karsil at a dose of 50 mg/kg. Based on preliminary research results, it was found that Soforaflavonoloside does not lag behind karsil and even slightly surpasses a comparable drug in hepatoprotective and choleretic properties. This suggests that the substance under study requires a more extensive in-depth study of the effect of sophoraflavonoloside on the normal and impaired functioning of the liver.

Many works have been published in the world literature, the main object of study of which are women of reproductive age, including healthy women, women with recurrent miscarriage and infertile women undergoing assisted reproductive technologies. Most of these studies have shown a significant association between the presence of thyroid autoantibodies, infertility, and an increased risk of miscarriage. Adequate levels of circulating thyroid hormones (thyroid) are essential for the normal functioning of the reproductive system. This article examines the relationship of miscarriage with autoimmune thyroid diseases (ATT) and the circulation of antiphospholipid antibodies (APS).

Adrenal insufficiency in patients with a severe course of viral hepatitis is common and leads to unstable hemodynamics, liver failure and a high mortality rate. In many cases of severe course of viral hepatitis there is an increase in cortisol in the blood. Our studies assessed the level of cortisol and transcortin in patients with viral hepatitis. It was revealed that lack of functional activity of endogenous cortisol depends on decrease of transcortin level in blood.   Free fraction of cortisol was more elevated in patients with HDV and HBV infection in comparison with patients with HCV infection and this increase was accompanied by lowering of transcortin in the blood.
